G,day all,
Watching nat geo on austar at the moment about Challenger. These guys should have been charged with manslaughter at the very least. They were warned not to launch because of faulty o-rings. Check it out.

Both shuttle disasters were avoidable and I am in no way offering a defence for NASA, but I do admire the way they 'air their dirty washing' and come clean. Something I am not at all confident that Russia, China or even ESA would do.
NASA, when they lost their Mars probe, actually admitted it was caused by two teams using different units, metric and imperial. No loss of life but still refreshing honest.

Yeah, when they do make embarrassing mistakes they seem to come clean. They did this when they launched the hubble. There was a problem with the optics, so a repair mission was launched and they fixed it. The initial problem was something really stupid (I can't remember).
